To provide an additional resource to HealthNow employees to facilitate smoking cessation in support of a smoke free workplace environment to promote good health.
APPLICABILITY This rule/policy applies to all HealthNow employees. Management (Supervisor and Above) Salaried/Exempt Hourly/Non-exempt (excluding bargaining unit) Bargaining Unit (Buffalo) POLICY/RULE
HealthNow will provide for co-pay reimbursement for Chantix and Zyban (buproprion SR) and/or the cost of up to eight (8) weeks of acupuncture, hypnosis, or laser treatment for the management of smoking cessation. To take advantage of this benefit, employees must purchase prescriptions through mail order or as specified by the employee’s health coverage. Acupuncture, hypnosis or laser treatment is to be provided by a qualified practitioner (for example, a licensed psychologist, counselor or acupuncturist).
PROCEDURE
HealthNow employees with a prescription for the medication Chantix or generic Zyban are eligible for reimbursement of their prescription drug co-pay. Employees are required to obtain this prescription from Medco mail order pharmacy or as specified by the employee’s health coverage. In most cases this would be a three month supply but may be extended to up to a six month supply. Coverage and co pay reimbursement beyond a three month supply will be managed on a case to case basis. Members must submit their pharmacy receipt to the Pharmacy Services after completion of the treatment with Chantix or generic Zyban. (i.e. after end of the three or six month supply).

VI. RESPONSIBILITIES
Upon completion of treatment, employees are responsible for providing Human Resources with receipts verifying prescription co-pays and/or payment and dates of service for practitioner services. VIII. DISTRIBUTION
This Policy and Procedure shall be distributed to all managers for communication to staff within their respective span of control. In addition, this document shall be available to all employees via the Intranet.
REVISION
It shall be the responsibility of the VP Human Resources (or designee) to review this Rule/Policy and Procedure on a regular basis and make revisions as appropriate. All rule/policy and procedure changes may be completed without prior notice. (As a policy applies to Bargaining Unit employees, the current agreement between HealthNow and the Office and Professional Employees International Union will be followed.)
